This program can help you evaluate the cost-effectiveness of in-home care and can be a great way for your family to get closer.

Maryland should reinstate a provider home-care program

The state recently ended the independent provider home-care program. This gives individuals more control over their finances and care. However, this decision will cause more harm that good, not only to Medicaid beneficiaries but also to home care aides who are covered by the program. While the original state projection was that cancellation of the program would lead to a significant increase in costs, Maryland Center on Economic Policy found that it would only cause a small increase.

Maryland Medicaid members may have experienced negative consequences from the removal of self–directed care. The state was trying to shift the burden of compliance with federal law from private employers to its citizens. But self-directed care participants might experience better outcomes than those who are provided home care services by agencies.

LHCSAs are licensed homecare services agencies

Licensed home care services agencies, or LHCSAs, provide nursing and personal-care services to clients in the comfort of their homes. Many of these companies are contracted by Medicaid and Medicare to provide such services. They might also contract with other agencies for certain services. These agencies are regulated by the New York State Department of Health, and are required to follow certain state regulations.


Before an applicant can apply in order to be granted a license, DOH must receive a detailed business plan detailing anticipated expenses for two months. The plan must include information about salaries, office space requirements, utilities, supplies, as well other operational expenses. In addition, an applicant must submit a most recent balance sheet.

While home visits using video can complement or replace regular visits, this technology is not an affordable substitute. For video visits to be effective, they must be capable of significantly reducing the time a provider has to spend with each patient. A patient with mild disability will experience a decrease in time of around 80%. This reduction should be smaller for patients with average disabilities: it should be 6% or less.

Recent studies have shown that inpatient and outpatient home care are more affordable than traditional inpatient. One study compared six Philadelphia hospitals to see how home care compared to hospital care. Study found that patients who were treated at home saved on average $6,500. The authors suggested that Medicaid, Medicare and other payment options could expand and make home care more affordable.
